CHIP Notice
If you or your children are eligible for Medicaid or CHIP and you’re eligible for health coverage from your employer, your state may have a premium assistance program that can help pay for coverage, using funds from their Medicaid or CHIP programs. Click here to view the CHIP Notice.
Medicare Creditable Coverage (Part D) Notice – As of 10/2024
Omaha Track is required by the Centers for Medicare and Medicaid Services (CMS) to provide the Creditable Coverage Disclosure Notice to all employees and their dependents covered under the Omaha Track, Inc. Medical Plan who may be eligible for Medicare. When you or a family member becomes eligible for Part D (Medicare’s prescription drug benefit), you need to enroll at your earliest opportunity. Not enrolling for Part D when first eligible may result in a late enrollment penalty. If you and your family are not currently eligible for Medicare Part D, but will become eligible in the next year, utilize this notice.
